UnitedHealthcare Implements Medical Necessity Reviews for Some Musculoskeletal Procedures

UnitedHealthcare is implementing medical necessity reviews for certain musculoskeletal surgeries rendered in an outpatient hospital (including of the elbow, wrist, ankle, hip, knee, or shoulder, as well as foot surgeries for bunions and hammertoes). The outpatient hospital site will be determined to be medically necessary if certain criteria are met.

If the outpatient hospital site of service is determined not to be medically necessary, the claim will be denied and the member is responsible for the cost of the service as long as the hospital obtained the member’s written consent to be billed in accordance with UHC’s protocols, prior to the service being rendered.

New UnitedHealthcare Vision Products Available

UnitedHealthcare recently released two new vision products, including an enhanced second pair benefit and a computer glasses benefit.

The enhanced second pair benefit is an in-network benefit that gives employees additional covered lens options to create a customized second pair of glasses. Employees choose from the combination of covered lens options which include Standard and Premium anti-reflective coatings, polycarbonate lenses, standard progressives, and photochromic to create their second pair. Examples include sports glasses, prescription sunglasses, computer glasses and backup glasses.

The computer glasses benefit provides coverage for prescription glasses that protect against the blue light emitted from eleectronics. The benefit includes 3 different allowance options for the client to choose from and includes Premium and Platinum anti-reflective coatings (options include blue light protection) covered in full.

Medical Mutual recently released a new Sold Case Pre-submission Review form that is required for groups to become part of COSE MEWA. Find the form here or at cosemewa.com.

The form must be filled out completely and accompany all sold case submission documents to ensure timely processing.

FormFire Instructions for Enrolling Employees in the Anthem SOCA Lumenos HSA Plan

FormFire recently released a notice with instructions for enrolling employees in the Anthem SOCA Lumenos HSA Plan. See below.

Notice from FormFire:

Employers that opt to offer the Lumenos HSA Option 7 plan require special instructions to assign the plan to the group and elect employees to the plan in FormFire. This is due to the plan having an aggregate deductible and only two tier levels: single and family.

Medical Mutual Has Extended The Mid-Size Group Volume Bonus Program

Medical Mutual has extended their Mid-Size Group (51–99) Volume Bonus Program for the period from July 1 to December 31, 2019. This program pays a per-employee per-month bonus on 51–99 group contracts with effective dates through December 31, 2019, as follows:

Tier Mid-size Group Book of Business Bonus Amount
1 75-199 contracts $4 per employee per month
2 200-349 contracts $7 per employee per month
3 350-499 contracts $10 per employee per month
4 500+ contracts $13.50 per employee per month

As a reminder, the Small Group (1–50) Volume Bonus Program is in effect through Dec. 31, 2019.

UPDATE: New IMQ From Aetna Funding Advantage

Aetna Funding Advantage has restructured their Individual Medical Questionnaire (IMQ) to make the process easier. The new IMQ can  now be accessed on Producer World.

Aetna clarified conditions and created high-level buckets of conditions to make it easier for members to review and answer questions on their health history.

You can start using the new small group AFA IMQ today. Starting September 1, 2019, Aetna will no longer accept the old IMQ.
